CoorsTek Wellbeing Program
The CoorsTek Wellbeing Program exists to educate, motivate, inspire, and support you in leading a healthy lifestyle and improving your overall wellbeing. The goal is to provide employees with the necessary resources, tools, and knowledge to live a healthier life. The CoorsTek Wellbeing Program takes a holistic approach and includes physical, mental, financial, and spiritual wellbeing.
All CoorsTek, Inc. employees are eligible for all wellbeing programs. All other employees, please see your local Human Resource Business Partner to determine if you are at a qualifying location.
CoorsTek Wellness Hours are designed to encourage and support employee wellbeing by removing barriers for getting recommended health screenings. Employees are provided two paid Wellness Hours annually to obtain a preventive medical exam, including: annual physical, cancer screenings, vaccinations and other appropriate preventive screenings. Talk to your doctor about what tests are recommended for you and what is covered as preventive.

The Wellness Hours DO NOT IMPACT your existing or accrued PTO. You must submit a time-off request to your supervisor. Please take the Wellness Hours Physician Form to your doctor for completion, then submit the form to your HRBP.

Wellness Reimbursement Program
The CoorsTek Wellness Reimbursement program is designed to help encourage and support healthy lifestyles. It covers a variety of wellness expenses such as gym memberships and nutrition counseling. Up to $300 in eligible expenses can be reimbursed annually. To be eligible, you must complete a Health Screening by 9/30/2022.
To satisfy the Health Screening requirement, you may attend a Quest onsite event, visit a Quest lab or see your own doctor. If you plan to visit your own doctor, please see the Physician Form Instructions below. Please visit the Quest site (My.QuestforHealth.com) to obtain a Physician Form and log your visit or to make a Quest lab appointment.
To be reimbursed, please email receipts along with a Wellness Pay and Deduction Adjustment Form to firstname.lastname@example.org. 2022 reimbursement requests are due by 12/16/2022.

What is Mental Wellness?
Mental wellness is a state of wellbeing and can encompass multiple dimensions: mental, emotional, social, and psychological. It is often associated with the concept of psychological well-being, which includes self-acceptance, growth, purpose, autonomy, environmental mastery, and positive relationships.¹
Mental health and behavioral health visits are covered the same as any other medical issue under the CoorsTek Aetna and Kaiser medical plans. Coverage is subject to the deductible and coinsurance. Please visit www.CoorsTek.com/Benefits for more information.
The 988 Suicide & Crisis Lifeline

The CoorsTek Chaplains provide confidential, personal or pastoral care in numerous ways and are available to employees and any immediate family members. Trained chaplains provide emotional and/or spiritual support in areas of marriage, family, parenting, other relationships, substance abuse, health/illness, and more. Chaplains visit the worksite to build relationships, but will visit hospitals, homes, perform weddings and funerals as requested. This benefit program is voluntary, paid for by CoorsTek, is a confidential service, and available 24 hours a day, 7 days a week via a downloadable App.

Screening & Preventive Care
Preventive care is intended to keep you healthy and detect certain diseases early. Kaiser and Aetna members receive preventive screenings, immunizations and flu shots at no charge.
CoorsTek employees are entitled to two paid wellness hours when obtaining preventive care or immunizations. Learn more about wellness hours.
CoorsTek employees may visit a Quest lab any time during the year for a screening lab test at no charge. Each fall, employees may participate in a Quest on-site event at their workplace for a medical screening and/or flu shot.
